Please complete this form if your child needs to keep an inhaler or Epi-Pen in school, or needs to be administered prescribed or non-prescribed medicine during the school day 

All medicine, whether prescribed or over the counter, must be in the original container and needs to have your child's name on it and handed in to the school office.
